California's Self-Determination Program provides eligible regional center consumers with an alternative way to arrange and direct their services and supports.
We Can Help You
Learn about person-centered planning, identify hopes and dreams, define meaningful goals, identify strengths and support needs, explore community and natural supports, identify potential services and resources, prepare for planning meetings, develop a person-centered plan, communicate preferences, and review progress.

Services We Offer
Independent Facilitator Services
Independent Facilitator Services provide personalized guidance and advocacy throughout the Self-Determination Program (SDP). We help individuals and families understand their options, prepare for Regional Center meetings, develop Person-Centered goals, support the development of the Individual Budget and Spending Plan, identify services and providers, and navigate the transition into SDP. We continue to provide ongoing support to help ensure services remain aligned with the individual’s needs, choices, goals, and vision for the future.
Person-Centered Planning Services
Person-Centered Planning Services put the individual’s goals, preferences, strengths, needs, and vision for the future at the center of the planning process. We work closely with individuals and their support teams to identify what is important to and for them, explore needed services and supports, and develop a comprehensive Person-Centered Plan that reflects their unique needs, choices, and desired outcomes.
